Based Services

Home and community-based services (HCBS) provide opportunities for Medicaid beneficiaries to receive services in their own home or community. These programs serve a variety of targeted populations groups, such as people with mental illnesses, intellectual disabilities, and/or physical disabilities.

Adult Day Care

Adult Day Care Programs provide a structured program of therapeutic, rehabilitative, social and leisure activities in a monitored setting. The programs offer supportive services to the participant, as well as to the family, by providing care and supervision in a protective environment during the day. An adult day care program is a group program designed to provide care and supervision to meet the needs of five (5) or more functionally impaired adults for periods of less than twenty-four (24) hours but more than two (2) hours per day in a place other than the adult’s own home. An Adult Day Health Care is a medical model program certified to provide Medicaid reimbursed services to Medicaid eligible participants as required by the MO HealthNet Division. An Adult Day Care program provides social activities and is not required to provide medical services. As a social program, an Adult Day Care participant is not eligible for Medicaid reimbursement. Hospice

Hospice is a special way of caring for people who are terminally ill, and for their families. This care includes physical care and counseling. Hospice care is given by a public agency or private company approved by Medicare. It is for all age groups, including children, adults and the elderly during their final stages of life. The goal of hospice is to care for you and your family and to help you make the most of the last months of life by giving comfort and relief from pain.
